ORDERING MERCHANDISE
Send orders in promptly by mail. Order by style No. only, do not send description.
Orders taken within 15 days prior to customer use should be phoned or faxed.

A complete tuxedo consists of a coat, pants and shirt. (Cummerbund & tie optional at no cost). If your order does not consist of these (3) items, you will be charged full price for each individual item, as noted on the price list! (This may be at a higher cost to you).

PROM POLICY
To better serve your needs and to be able to ship earlier, we ask that you get 2 choices on all prom orders.

LAST MINUTE ORDERS
If a last minute order is not received on time or any order is refused, customer will be responsible for all shipping costs regardless of shipping method.

SHIPMENTS

Free Rental Shipping Policies
Ptaks's provide FREE outbound ground shipping to UPS Ground Zones 2,3,4 provided the order is placed 10 days before date of use. Ten days prior to date of use starts the day before the day of use to establish free shipping. All weekend/Sunday use dates count from friday. All zone 5,6 and 7 customers pay for outbound shipping costs. Customer request for two day or next day air shipping will be at your expense. We cannot guarantee same day shipping on new orders placed after 12:00 P.M. Ptak's pays return ground shipping on all rentals via UPS authorized Return Service.

Small Order Shipping Policy

Any weekly invoice of $25 or less will be charged both outbound and inbound shipping cost.

RETURNING MERCHANDISE
Your customer must return garments to you on Monday or the following day after use.

Returned merchandise must be received by us no later than the Friday following date of use. Additional (1/2) half week rental will be charged on returns after the time allotted. Additional delays will result in billing for replacement costs.

RE: REPLACEMENT COSTS
Once billing for replacement costs, of merchandise not returned, has occurred, merchandise must be returned to us within 30 days of date of use for a credit to be issued!

After the 30 day grace period, no credit, of any kind, will be issued!

CANCELLATIONS
Orders cancelled during shipment process will incur a service charge of $10.00 per unit. Once you receive a garment that will not be used, a Ptak's Not Used Ticket must be completed, attached to the unused garment and returned promptly for a credit to be issued. A $10.00 service charge applies.

SERVICE CHARGE RENTAL ITEMS

RETURNED - NOT USED
Complete Tux - $10.00; Coat Only - $5.00; Pants - $3.00; Skirt - $3.00; Vest - $2.00; Shoes or Boots - $2.00; Shirts - $1.00; Cummerbunds - $1.00; Tie/Button Cover - $1.00.

SERVICE CHARGE - NEW PURCHASED

ITEMS RETURNED
The grace period is 15 days from the date you receive your order provided the merchandise has not been used, abused, or altered in any way. Original joker tags must still be intact and in place to receive credit. Any claims beyond that limit will be subject to a 10% service charge.

DAMAGES-LOST MERCHANDISE
Check every return for damages. Damages will be charged to your account at replacement costs. It is your obligation to collect from your customer. Damage waiver for your account may be obtained by completing our damage waiver form.

OPTIONAL DAMAGE WAIVER POLICY
Customer agrees to pay the sum of $4.00 for damage waiver per unit. Ptak's agrees to waive claims against customer resulting from unintentional damage to the rental garment during the rental period. This agreement does not cover intentional destruction, or loss, or theft regardless of customer negligence.

BILLING
All invoices must be paid within 15 days of the date of use! A 1-1/2% late fee charge per month on any invoice past due, this is an annual percentage of 18%. The provisions in this contract shall be governed by the laws of the state of Pennsylvania. All interest charges must be paid.

Any disputes must be made in writing, within 30 days of the invoice!!
Any invoice past due over 30 days, all orders will be shipped C.O.D.., with past due balance attached.
All new accounts will be serviced on a C.O.D.. basis until credit is approved by Ptak's Formal Wear. We must have a completed and signed credit application on file.

EXCHANGES (SHIPPING)
Exchanges will be made at no cost when it corrects a shipping error. If your customer or store personnel made the error, you will be responsible for 1 /2 the shipping cost. No-cost exchanges do not include adult coat single size adjustment or sleeve or pant length adjustments. Shoes 1/2 size smaller wide to medium width. We take pride in shipping as ordered.
